L1: IRS Form to file

I followed this web site many times in Jan of 2006 to help me figure out how to set up a 72T account for myself in Feb. A very deeply informative discussion. My memory tells me that this year after receiving distributions from a 72T account, I need to file under, not under a 1040 form, but a 5329 (?) form.
My accountant gave a DRAFT copyunder a 1040 form.
Can I file under 1040 or do I need to file in an alternate way?

L2: IRS Form to fileThe 1040 is the basic form which you will file and any other forms support in more detail the income and deductions that are all listed on the 1040.
The 5329 is a minor form that is used for reporting penalty taxes or claiming an exemption from a penalty and is attached to the 1040. You will need it to claim an exception from the early withdrawal penalty if the 1099R your IRA custodian issues does not show a code of “2” in Box #7.
