I don’t necessarily want to appear to be technical; however, you would not use a life expectancy table with the annuitization method. Instead, you would use a mortality table to build an annuitzation factor (usuall from a spreadsheet). The right mortality table to use is an appendix to Revenue Ruling 2002-62. I think it is the same one you will find in the final 1.409(a)(9) regutations but I not 100% sure.
